With it being 13 years old , not knowing mileage / service history or hearing the noise , hard to say . Could check transmission oils but may be too high a gear for your low speed or DMF .

Milage - 120k
Full service history up until COVID
Last MOT was clean bill
Next is due end of the month
It's just while in 5th gear it feels like you're pressing on and off the accelerator
Is changing changing DMF an expensive fix?

What you describe would have nothing to do with or part of the MOT s , DMF is an expensive repair .There are so many possible causes it really is hard to advise without checking the vehicle . One possible cause could be MAF sensor needing cleaned / replaced
