2006 Ford Focus judddering on acceleration in high gears

when cruising in 5th or 6th gear at 50mph and you accelerate, the car judders as though its missing, and front end feels like its going over a lot of small bumps in the road, engine pulls well and has plenty of power, ease off the accellerator and the juddering stops, any ideas where to look for the problem

First check for fault codes,2nd has vehicle been maintaned correctly i.e. fuel filter renewed,maybe worth trying a filter if no codes present.
