hi i have a problem with reluctance to pull off when cold and juddering when rev counter reaches 3000 anyone got any ideas please

I have just had the codes read on my car and its come back with code po400 what is this and how much will this cost me give or take few quid

When u start the car from cold u get a strong smell of unburnt diesel also when the car is warm u sometimes get the same strong smell why ??
