Drops out of 4th gear and seems to be stuck in 3d

When I drive of from cold in the morning, the automatic transmission changes into top gear at about 42mph then you here a clunk and the gearbox changes down a gear , as this happens I noticed both the S and ice sign briefly come up on the dash next to the drive sign travel 12 miles to work changes up and down but not into top gear. Left at work all day on the way home all fine again the perfect gearbox, any idea anyone.
