2004 Renault Scenic starting problems - injection fault

Year: 2004
Variant: 1.5 dci

On turn over I get a warning light Injection Fault.
Diagnostic said Glow Plugs and Relay faults. I have since changed both the glow plugs and the relay as well as cleaned injectors and changed the fuel filter. But the problem is still the same. If I do manage to get the car started then it runs nice and smooth unless I accelerate hard in high gear, then the injector fault warning comes up again, but no loss of power noticed.
3 out of four glow plugs get red hot and one only warm (No4).
I have not been able to trace a problem with No4, as yet.
Does anyone know where I look next. This appears to be a common problem but I can find no answer on any forum and would appreciate some helpful advice.

Go back to whoever read the codes and get them to do a live data fuel pressure test.
preferably on a 'scope to make sure you have at least 200 bar on cranking and the fuel pressure flow when the light comes on.
But as a get you started dodge,dis-connect the air flow meter and spin it over.
Not that I'm suggesting the AFM is at fault,my main suspect is the fuel injection pump.but would need to see the live data first.
