Reverse and 5th gear not working

when i put the car into reverse it still goes forward as in 4th gear and when i put it into 5th gear it goes into 3rd

i would say your gear linages have either worn out and want replacing or adjusting if they can be adjusted but would only have this looked at by a garage of course
