2011 Ford Mondeo jerking when automatic gears are changed, jolting and shaking of the car when i come to a stop and engine malfunction light when i accelerate my car harder than it wants to go or when driving for long periods of time

My car hesitates to change gears when I’m driving and can jolt which I think may be a transmission problem. But also I can’t accelerate my car faster then it wants to drive. For example up hills it drives really slow and when I force it it’ll harshly change gears and rev really hard when doing so. Sometimes during this my engine malfunction light comes on, not the actual engine light. Or when driving long distances my engine malfunction light will randomly come on. When I force my car like explained before. When I come to a stop my car with shake, jolt and sometimes make a slight tapping noise that comes with it, sometimes it’s more severe than other times.
