2000 Ford Galaxy loss of 4th gear, irratic rev counter

Variant: 2.3 ghia automatic

Car starts with no problems, when driving rev counter bounces about, gearbox will drop into 3rd and not go back to 4th. At times rev counter max's out and gear box appears to not drive. Stopping and starting again can some times clear fault.Any help please.

Best to get a diagnostic scan and see what shows .
