2004 Ford C-MAX grinding from front of car when pulling away

ive notices that over the passed week there has been a crunching noise or grinding noise coming from the front of my car when i pull away the car is losing power at low speed and is very jumpy around 40_50 mph it has only got 40.000 on the clock and is upto date with all of its servicing

sounds like a fault with the gearbox or the torque converter which is the same as a clutch on a manual
