Quite a list but it all could be connected,
1st thing to do is get the EGR of for evaluation/operation.
A faulty EGR can cause a number of symptoms including the miss fire and intake faults.

No way to really say,
Central London would be sky high,
In the sticks a lot cheaper.
Best bet is call you local indie and ask them for a price to check out and possibly replace the EGR.
