Blocked exhaust

My car has blown a pipe of the turbo twice in the last few weeks. It tunrs out that the exhaust was full of soot. I took the car to the main dealer in June 09 who apparently regenerated the system and topped up the diesel patriculate fluid which is supposed to burn off the soot at an extremely hight temperature. So why if everything is supposed to be sorted did my exhaust block? Grateful for any thoughts?

suspect diesel particulate filter(cat) broke up and got sent into the rest of the exhaust system, caused enough back pressure to blow pipes off turbo... theres one senario...
