1997 Ford Mondeo rpm problem

Car: Ford Mondeo
Year: 1997
Variant: LX 2.0 16V
Categories: Running Rough, Starting & Power Loss
At first I thought the clutch was going but tried doing about 30mph in 5th gear and then dropping it into 3rd and accelerating to see if there was any slip? What I found was the revs struggled and could hardly make it passed 4000rpm and after that it felt like the car was juddering abit while doing a constant speed , what could be the problem?

Check plugs , plug leads . Cant remember when it changed and if that year had a timing belt or chain . If timing belt and not been done for quite a while , may be worn and jumped , throwing out timing .But check easier things first .
