2002 Ford Mondeo "groaning" steering

I've noticed recently that the steering is "groaning" and it feels like it's grinding when turning. Could this be as simple as a lubrication issue or could it be something more severe? The steering does feel a little heavier than I would expect but not signficantly.
Any ideas?

check and top up your power steering fluid and check for leaks
