2004 Ford Mondeo struggling/holding back/loss of power

whenever i put my foot down my car seems to judder and stuggle to gain any power! it seems to mainly do this at 3-4 thousand revs but even when its sitting at 2 thousand it will still do it..iv recently replaced my plugs and i done the plugs,coil pack and leads about 4 months ago, i have noticed a slight crack in the leads but this surely cant be just the problem?

try some insulation tape around the cracked leads to start with, if this helps change the lead.
