1999 Ford Focus high revs with no acceleration

Car: Ford Focus
Year: 1999
Variant: 1.8 Zetec

When the car is stationary the revs are low as would be expected and the car generally drives fine. However, when slowing down if I hold the clutch down the revs stay at about 2000 until the car comes to a stop.

It also appears to do this when changing up gears, but is less noticeable. An additional note, I don't know if this is a factor, when putting the car into reverse I cannot have the clutch down fully, it has to be raised an inch or two before it will go in.

throttle position sensor or a leak from a vacuum or breather pipe.
