1996 Ford Escort grinding noise

when i am driving i sometimes hear a really bad grinding noise and the van swerves a little bit iv looked underneath but cannot find anything wrong any help please.

Get it jacked up and check for any excessive movement at wheel .Possibly wheel bearings or disc pads .
