Loud grinding noise

My car is making a loud horrible grinding sort of sound in every gear except 3rd. Also becoming a bit difficult to get into gear. Sounds perfect in idle also to the biting point. It started in 4th and 5th. Then was 1st and 2nd now it's all 4. Pls help. :)
